I formerly known as Adeyemi, Adebola Adekemi now Mrs Adefioye, Adebola Adekemi hereby declare that according to my birth certificate, I was born on 9th April, 1980 and on my Passport there was a mistake in my age.
This declaration is now necessary to facilitate the correction of my age and change of name in my Passport.
All former documents remain valid.
